This form is for ELEMENTARY VBS.

Elementary VBS is designed for children who have already attended elementary school.
Kids should be entering 1st-6th grade in the fall.
If your child has not attended Kindergarten yet, they should attend Pre-K Week.

We do our best to accommodate requests, but do not make guarantees. Please understand for safety reasons we cannot pair a young elementary students (i.e. 1st/2nd grade) with an older elementary student. (i.e. 3rd/4th grade).

I also assume the responsibility for the payment of any such treatment. I hereby grant permission for my child to be photographed, voluntarily and without compensation, understanding that the same is intended for publication by print media, online media, newspaper and video purposes.
